Extraordinary Caregiver Award Program

Honoring Dedicated Caregivers

At Summersett Funeral Home and Cremation Center, we hear every day about the wonderful work that caregivers provide the families of Salisbury and Rowan County.

To honor these dedicated people for the great care and comfort they bring to the patients and their families, Summersett’s has created the “Extraordinary Caregiver Award” program.

We realize that it takes special people to work in hospice and there are wonderful caregivers throughout Salisbury and surrounding communities. Therefore, this program is open to all Caregivers that serve the Salisbury & Rowan County area, whether they are for-profit or not-for-profit. Our goal is to honor great caregivers.

If you would like to recognize a caregiver for their dedicated service excellence, you can fill out the nomination form here. Just mail it in or drop it off at Summersett Funeral Home, 1315 W. Innes St., Salisbury, NC 28144.

Eligibility
We know it takes a special person to work or volunteer as a caregiver. Any hospice employee, volunteer caregiver, hospital nurse, etc.. who provided care or support directly to a patient or their family is eligible to be nominated for the award.

  • Nurses and nurse aids
  • Social workers
  • Bereavement staff and chaplains
  • Volunteers
  • Anyone who works directly with the patient and their family
  • Office support staff

Caregiver Awards Ceremony
Each month, one of the nominees submitted for that month will be selected to win a special award ($100) — just a little something special to say “thank you.” The Program Director and the Program Coordinator will make arrangements to visit that hospice personally to deliver the award and take a photo of the award winner. These winners will also be promoted on the Summersett Funeral Home website and Summersett Funeral Home & Cremation Center Facebook page.

Then, once per year Summersett’s will hold a special Caregiver Awards luncheon for all of the award monthly winners. Each will receive a small gift as a token of appreciation for the wonderful care they provide. One of the nominated caregivers will be selected by our selection committee to win a special scholarship award for their dedication and service excellence.

The scholarship is intended to enhance the knowledge and skills of caregivers within the hospice setting. The grand prize winner will have an opportunity to use their scholarship for a local continuing education program or conference, as well as for career-enhancing certifications.

Nominees are only eligible to win the grand prize once. However, nominees are eligible to win the monthly award more than once.

How to nominate

You can nominate a hospice caregiver or co-worker who you believe exemplifies what it means to be an extraordinary caregiver. If you would like to nominate someone, please complete the nomination form.

Nomination brochures are also located in many businesses around town for easy pickup and submission.
